It can help participants manage disability-related health needs while supporting greater independence in everyday life.<\/p>\n<p>At Aspire Hub, our Community Nurse <strong>Angelique<\/strong> works directly with participants to provide nursing support based on their individual needs.<\/p>\n<h2>What is NDIS community nursing?<\/h2>\n<p>NDIS community nursing provides eligible participants with disability-related health supports outside of a hospital or traditional healthcare setting.<\/p>\n<p>The NDIS may fund health supports where they relate to a participant\u2019s disability and meet the NDIS funding criteria. You can learn more about <a href=\"https:\/\/www.ndis.gov.au\/participants\/using-your-funding\/other-types-support\/what-disability-related-health-support\">disability-related health supports through the NDIS<\/a>.<\/p>\n<h2>What can community nursing help with?<\/h2>\n<p>Depending on the participant\u2019s individual needs, community nursing support through Aspire Hub can include:<\/p>\n<ul>\n<li>Health assessments and tailored care plans<\/li>\n<li>Wound care and management<\/li>\n<li>Medication administration and reviews<\/li>\n<li>Chronic condition and diabetes management<\/li>\n<li>Continence assessment and support<\/li>\n<li>Coordination with GPs and other providers<\/li>\n<\/ul>\n<p>You can explore Aspire Hub\u2019s full range of <a href=\"https:\/\/www.aspirehub.com.au\/ndis-housing-services\/community-nursing-services\/\">Community Nursing Services<\/a> to learn more about the support available.<\/p>\n<h2>Who can benefit from community nursing?<\/h2>\n<p>Community nursing may be suitable for NDIS participants who require disability-related nursing support as part of their everyday life.<\/p>\n<p>The type and frequency of support will depend on the participant\u2019s individual needs, circumstances and NDIS plan. The NDIS provides further information about <a href=\"https:\/\/www.ndis.gov.au\/participants\/using-your-funding\/other-types-support\/what-disability-related-health-support\">what disability-related health supports may be funded<\/a>.<\/p>\n<h2>Meet Angelique, our Community Nurse<\/h2>\n<p>Angelique is Aspire Hub\u2019s Community Nurse, providing nursing support to participants in their homes and communities.<\/p>\n<p>She works directly with participants and their wider support networks to understand their needs and provide nursing care tailored to the individual.<\/p>\n<h2>What can a community nursing visit involve?<\/h2>\n<p>Every participant is different, so community nursing visits are based on the individual and the support they require.<\/p>\n<p>This could involve providing agreed nursing supports, completing health assessments, managing wound care or medication, developing care plans or coordinating with the participant\u2019s other healthcare providers.<\/p>\n<p>Aspire Hub currently provides Community Nursing Services across <strong>Ipswich, South Brisbane and the Gold Coast<\/strong>.
